Financial Policies

Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all the costs before treatment is administered. Charges are payable at the time of treatment. We offer the following payment options:

You can choose from:

• Cash or check: We are happy to offer a 10% prepayment courtesy for major restorative treatment paid in full at the beginning of treatment.
• Credit cards: For your convenience, we accept Visa, MasterCard, and Discover credit cards.
• Payment Plans: For major restorative treatment, we offer the convenience of up to three equal payments for treatments over $300.00.
• Extended Payment Plan: For those patients who desire a monthly payment plan, we offer CareCredit payment option. This payment option is a fast and simple way to schedule your treatment and feel the comfort of knowing that you can make interest free payments for up to 12 months. Pay no up-front costs, and no annual fees. And if you wish to choose an extended payment plan, we can discuss the financing with you.

Other Charges and Fees

• A finance charge of 1.5% will be assessed to account balances that are 90 days overdue.
• A charge of $30.00 will be assessed for returned checks.

Insurance

For patients with dental insurance we are happy to work with your carrier to maximize your benefits and directly bill them for reimbursement for your treatment. We are participating providers with United Concordia, MetLife, Security Blue, Freedom Blue, CHIP Program, Guardian, and Delta Dental. While making every effort to provide you with your co-pay and deductible amounts prior to your care, we cannot be expected to know all of the details for every insurance contract. All dental insurance contracts are agreements between your employer, you, and the insurance company. If you have specific questions concerning your dental coverage you should contact the Human Resources department at your place of employment.
